Client & Location Information:

Join a committed healthcare team based centrally on the Gold Coast, where your role as an Aged Care On-site Pharmacist (ACOP) will be central to improving medication safety and outcomes for aged care residents. This role also includes occasional travel to a site in East Brisbane-with travel time and expenses covered.

Job Specification:

As the ACOP, you'll bring your expertise in medication management and pharmacotherapy to deliver integrated, person-centred care. Working closely with GPs, nurses, facility staff, and allied health professionals, you will support residents' health outcomes and build trust with their families and carers.

Your responsibilities will include:

• Daily monitoring of residents' medication and resolving medication-related issues promptly.
  
• Supporting medication management and communication during transitions of care.
  
• Acting as the sole on-site Pharmacist, embedded within the multidisciplinary team.
  

Skills Required:

To be eligible for this role, you must either:

• Have completed an APC-accredited aged care on-site pharmacist training program
  OR
  
• Hold a current Medication Management Review credential and commit to completing an APC-accredited aged care on-site pharmacist training program (or RPL process) by 30 June 2026
  

Salary & Benefits:

• $65 per hour plus super and all permanent entitlements.
  
• Stable weekday hours: 9am-4pm, Monday to Friday.
